- What time did the fall happen—and where?
- Was Mom rushing to the bathroom?
- Did she skip using her walker?
- Was she dizzy from a medication side effect?
- Was there a pet or a rug that got in the way?
- Did she fall trying to do something alone she shouldn’t have?
- Was a chronic condition unmanaged—for example, diabetes?

How We Intervene to Prevent Future Falls
1. Medication Review:
Did Mom take the right dose at the right time? Has a new prescription caused dizziness or balance issues?
2. Home Safety Audit:
Are there loose rugs? Cluttered walkways? Do we need to install grab bars, add a shower bench or a raised toilet seat?
3. Daily Routine Support:
Was she alone at a high-risk time? Consider adding morning, evening, or overnight care to assist with bathroom trips, dressing, and meals.
4. Cognitive & Physical Health Review:
Is memory decline or incontinence contributing to her fall? Should we introduce balance exercises for her to do with her care team?
